Nursing Info & Updates for the 2026-27 School Year

To ensure your student receives the support they need at school, all health-related documentation must be submitted to the school clinic each year and updated at the beginning of every school year. Log into SchoolCare to complete required steps for each enrolled child.

Severe Allergies, Asthma, Diabetes, and Seizure Disorders

If your student has a severe allergy, asthma, diabetes, or a seizure disorder and requires support at school, please provide the following:

• A completed and signed Action Plan from your healthcare provider for each condition.
• A completed and signed Hold Harmless Form if emergency medications will be stored in the clinic, including:
  • EpiPen®/epinephrine auto-injector
  • Inhaler
  • Glucagon
  • Emergency seizure medication

All prescription medications must be delivered in their original container with the prescription label attached.

If your student needs to self-carry emergency medication, please contact the school nurse for the required authorization form, which must be signed by the student's healthcare provider.

If clinic staff may need to communicate with your student's healthcare provider, please complete the Consent for Mutual Exchange of Information Form.

Other Medical Conditions

If your student has a medical condition that requires management during the school day, please provide the clinic with:

• Healthcare provider orders
• Supporting medical documentation

If communication between the clinic and your student's healthcare provider may be necessary, please complete the Consent for Mutual Exchange of Information Form.

Immunizations

Students must have current immunization records on file at school. If your student's immunizations are not up to date, please schedule the required vaccinations through:

• Your healthcare provider
• Hendricks County Health Department (317-745-9222)
• Most local pharmacies

Indiana Code 20-34-4 requires Avon Community School Corporation to exclude students from school if they do not have the required immunizations or documentation on file.

Medical or Religious Exemptions

For religious exemptions, complete and return the Religious Objection Form. If you have a medical exemption, please have your healthcare provider complete and sign the Vaccine Medical Exemption Form.

Required Immunizations by Grade Level

• 6th Grade: MCV4 (Meningococcal) and Tdap
• 12th Grade: Second dose of MCV4 (Meningococcal)

Updated Clinic Guidelines

Morning medications should be administered at home before students arrive at school.

Only medications that are necessary for a student to remain at school will be administered during the school day.

Parents/guardians must complete their student's SchoolCare enrollment and provide consent for administration of stocked over-the-counter medications, including:

• Ibuprofen
• Acetaminophen (Tylenol)
• Tums

Without SchoolCare consent, nurses will not administer these medications, and parents will not be contacted for verbal permission.

Vision and Hearing Screenings

Vision Screenings

In accordance with Indiana Code 20-34-3-12, vision screenings will be conducted for students in Grades 1, 3, 5, and 8. If you do not want your child to participate, please contact your school nurse.

Hearing Screenings

In accordance with Indiana Code 20-34-3-1, hearing screenings will be conducted for students in Grades 1, 4, 7, and 10.
If you do not want your student to participate, please contact your school nurse.

Back-to-School Health Checklist

Please submit the following, as applicable:

✅ Updated action plans for allergies, asthma, diabetes, and/or seizures
✅ Hold Harmless Form for medications stored in the clinic
✅ Self-carry authorization form (if applicable)
✅ Consent for Mutual Exchange of Information Form (if applicable)
✅ Healthcare provider orders for other medical conditions
✅ Updated immunization records or approved exemption forms
✅ SchoolCare enrollment and medication permissions

Completing these items before the start of the school year will help ensure your student's health needs are met safely and efficiently.
