Records Requests

Please use the links below to request your documents. We will make every effort to process your request within 24–48 hours during the regular school year. Please note that we operate on a balanced calendar.


Current Avon High School Students

All transcript requests must be submitted through Naviance. In addition, a blue transcript request form must be completed in the School Counseling office for your request to be processed. Please do not use the Parchment link to request your transcript.

Previous Avon High School Graduates

Replacement Diploma Information

To process your replacement diploma request, please provide the following:
  1. This completed Replacement Request Form 
  2. Official school transcript – must be mailed to Herff Jones in a sealed envelope.
  3. Payment – Check or money order made payable to Herff Jones:
    • $30.00 for a hard copy
    • $17.00 for a PDF copy only (no hard copy). If you request a PDF, please indicate this on the Replacement Request Form.

Please note that standard production takes approximately 6–8 weeks. Production may take longer in the spring due to college and high school diploma processing.
